    Title: 濫用藥物毛髮檢驗 LC-MS/MS 方法之開發與研究

    Abstract: 毛髮檢驗,利用其可向前追溯的特點,解析過去數月、甚至數年內的吸毒歷程,特別是對近年來盛行的新興新興濫用藥物,毛髮檢驗更能補足尿液檢驗檢測視窗不足的缺點,建立更嚴密的濫用藥物檢測網。在本計畫中,承接過去建構之了GC/MS之「濫用藥物毛髮檢驗系統」,依照藥物性質與實用性,發展了共36種濫用藥物與其代謝物,在 LC/MS/MS 的「系統性」藥物毛髮分析方法,並簡化系統,由最初的三個分析系統簡化成兩大分析系統。「系統一」針對國內常見毒品:包含安非他命、甲基安非他命、MDMA、MDA、MDEA、包括嗎啡、可待因、海洛因、6-乙醯嗎啡、K它命及Norketamine、PMMA與2C-B等13種藥物混合系統。系統二,包含:大麻類藥物(THC,THC-COOH),古柯鹼類藥物(古柯鹼,Benzoylecgonine),麻醉止痛類藥物特拉嗎竇,海洛因成癮之替代療法藥物(丁基原啡因,Norbuprenorphine,美沙酮,EDDP,EMDP,Naloxone,LSD),與苯二氮平類藥物(Diazepam,Nordiazepam,Oxazepam,Lorazepam,Nitrazepam,Temazepam,Flunitrazepam,Alprazolam,Triazolam) 等26種藥物及其代謝物。我們建立了藥物標準品在探討電灑法(ESI) 和大氣壓下化學游離法(APCI)之正負離子標準圖譜,與其藥物在離子阱式質譜儀之二次質譜圖譜,並以MRM方式進行靈敏度評估。此外,本研究也探討了多重藥物系統中彼此抑制之現象。隨後我們進行了藥物系統在LC上的滯留時間,結果發現其在質譜的重建後,每一藥物出現之滯留時間皆少於30秒,最後,我們進行了真實毛髮檢體之LC-MS/MS分析。在本次計劃收集到的35個煙毒犯之毛髮樣品,同時分別進行 LC/MS/MS與GC/MS之分析,結果呈現LC/MS/MS與GC/MS結果大致相符合之趨勢。而值的注意的是:LC/MS/MS測得的6-乙醯嗎啡/MOR比值皆高於GC/MS,因此LC/MS/MS方法在鴉片類藥物的毛髮檢驗上,有顯著之優勢。
    Hair testing is powerful with its natural advantage in drugs of abuse, can track forward several months age and even above of half a year. It can get for the court's evidence effectively, and more effective hindering the result of frightening emerges. Especially to the new developing drugs of abuse, hair testing can fill the short time window of urine testing and set up more compact able drugs of abuse network. In this project, we had developed e “System Hair Testing” from GC/EI-MS to LC/MS/MS for 36 drugs of abuse, including 「system 1」: 13 common drugs of abuse and its metabolites in Taiwan, (AP, MA,MDMA, MDA, MDEA, K, NK, MOR, COD, Herin, 6-AM, 2C-B and PMMA, and「 system 2」: 26compounds (cocaine, BZE, THC, THC-COOH, Buprenorphine, Norbuprenorphine Methadone, Naloxone, Pethidine, Tramadol, LSD and 9 BZD (Diazepam, Nordiazepam, Oxazepam, Lorazepam, Nitrazepam, Temazepam, Flunitrazepam, Alprazolam, Triazolam) Then, this research regards to estimate LC/ESI-MS/MS and LC/APCI-MS/MS by used direct injection Finally, we compared the performance of GC/MS, and LC/MS/MS for drugs of abuse hair testing in 35 real samples, and enable the hair testing method of inspection to be more complete system, accumulate experience to be able to more hair testing for the future.
