    Title: 降低台灣某醫學中心糖尿病人足部篩檢之異常率
    Reducing the Abnormal Ratio of Diabetic Foot Screening Test in an Outpatient Department of a Medical Center in Taiwan

    Abstract: 臨床上常發現糖尿病病人足部篩檢異常之比率偏高,而藉由足部篩檢及足部照護可有效預防糖尿病人足部病變,故本專案旨在降低某單位糖尿病人足部篩檢之異常率,以減少足部受傷機會,進而提升足部照護品質。本專案小組成員於2005年九月一日至2006年五月十日,針對糖尿病人以單股尼龍纖維及半定量音叉為工具,評估並觀察足部外觀,結果發現專案實施前之足部篩檢異常率為62.5%,原因是:病人不知要做足部照護、不瞭解足部照護衛教內容及不覺得足部照護重要。經實施增設衛教工具、修訂足部照護指導流程、實地進行足部照護指導及執行監測後,其足部篩檢異常率降至28%。本專案之實施,不但降低足部篩檢異常之比率,也讓糖尿病人確實執行足部照護工作,並提升足部照護品質。
    An abnormal to normal ratio of diabetic foot is frequently high in examinations performed in nurse-managed clinics. In order to prevent the foot complications in people with diabetes mellitus, appropriate evaluation and foot care are essential. In this project, we attempt reduce the abnormal to normal ratio of diabetic foot and improve the quality of foot care. To do this, we recruited subjects diagnosed with DM at medical center for a foot examination between Sep 1st 2005 to May 10th 2006. We assessed abnormality in diabetic foot with methods using monofilament, turning fork, palpation, and visual inspection. We found a pre-intervention ratio of 62.5%, possible a result of poor self-care of feet. After the intervention, which included a revision of the educational program, the examination tools and the skills of foot care, post assessment revealed that that the ratio had decreased to 28%. In conclusion, the implementation of this project not only reduced the abnormal to normal ratio, it also motivated patients to take better care of their feet.
