    Title: 特發性脊柱側彎對病患肺功能及心肺運動之影響

    Abstract: 本研究旨在瞭解特發性脊柱側彎對病患肺功能及心肺運動之影響。採描述性研究設計。針對中部某教學醫院骨科脊柱側彎矯正特別門診,經骨科專科醫師診斷為特發性脊柱側彎者之病患為脊柱側彎組,收案期間為94年3月1日至同年8月31日,共收案35人;接著上網公佈應徵身高、體重、BMI(body Mass index)符合組脊柱側彎組條件之大學生為脊柱正常組,收案期間為94年9月1日至同年10月31日,共收案21人。研究資料以SPSS 10.0套裝軟體統計及分析。結果顯示:(1)特發性脊柱側彎病患其好發部位與性別無相關;(2)肺功能檢測兩組在統計上具顯著差異(p < 0.05),且脊柱側彎組呈現明顯較低的情形;(3)心肺運動測試休息期、無阻力運動期、無氧閾值期、最大運動量期及恢復期期均在兩組間呈現統計上明顯差異(p < 0.05),脊柱側彎組顯著偏低。亦即是脊柱側彎組因脊柱側彎所造成脊柱扭曲導致胸腔變形,使得肺臟發育不良、胸廓彈性變差,以致呼吸供氧的效果欠佳、身體進行活動時容易喘息,限制了心肺運動功能,使呼吸供氧效果不佳,心血管循環系統不良。未來護理人員在照護特發性脊椎側彎病患時,可提供相關衛教,改善呼吸功能進而促進健康,提升生活品質。
    The purpose of the research was to understand the effects of idiopathic scoliosis on pulmonary functions and cardio- pulmonary exercises. Descriptive and correlation research methods were designed for pulmonary function tests and cardiopulmonary functions to compose the database from this hospital in the middle of Taiwan from August to October in 2005, including the patients diagnosed idiopathic sclerosis, 35 patients in total, in contrast with the 21 people recruited from the internet with the required conditions of body height, body weight and BMI (body mass index). The data of the selected 56 cases were analyzed by percentages, mean values and standard deviations by means of SPSS 10.0 for Windows to get descriptive statistics and then tested with t-test. The results between these two groups show: (1) no relation between the sex of idiopathic scoliosis patients and the vulnerable part for idiopathic scoliosis. (2) the significant difference in the pulmonary function in those two groups(p < 0.05)with the scoliosis group apparently showing lower pulmonary function. (3) the cardio- pulmonary exercise tests during rest, unload, anaerobic threshold, maximal exercise, and recovery periods in those two groups show significant difference (p < 0.05 with the scoliosis group also showing low function. The above findings show that the deformity of the costal cage makes lung development and compliance worse. Dyspnea compromises the oxygenation and cardiopulmonary function and even bed-ridden a person. To promote the life quality of patients with idiopathic sclerosis and the related nursing equipment to provide, these data are precious and useful.
