Herein, we report our nursing experience caring for a re-hospitalized patient with multiple sclerosis from March 24 to May 1 in 2009. Due to the repeated illness, patients with multiple sclerosis almost always have series of complications in the final stages of the disease. Using Gordon's eleven health functional patterns, the patient's nursing needs were evaluated by physical examination, observation, and attentive listening. The patient's nursing problems were identified as impaired function of physical and neuromuscular activities, risky infection and urinary incontinence, hopeless attitude due to health deterioration. We provided a rehabilitation program of range of motion (ROM) to maintain this patient's body activity and ideas to for her to prevent urinary tract infection. Otherwise, we also tried to enhance in her both physical and mental resistance to her illness. The patient gained self-care skills from our teaching and interaction.
多發性硬化症是一種慢性中樞神經系統疾病,突發的漸進式失能狀態,往往使患者身心遭受到強烈的打擊。本篇主要描述一位多發性硬化症個案因疾病復發反覆入院,造成個案身心衝擊的護理過程。筆者照護期間自2009年4月24日至5月1日,經由身體評估、觀察、傾聽等方式收集資料並利用電訪追蹤評值,運用Gordon十一項健康功能型態評估,確立個案有「身體活動功能障礙/神經肌肉受損」、「危險性感染/尿瀦留」、「無望感/與疾病導致身體狀況惡化未改善有關」等護理問題。護理過程中,針對個案問題提供個別性護理措施,給予主動及被動之全關節運動指導,以維持肢體活動功能,協助個案尿瀦留之處理並避免尿道感染;藉由同理心、傾聽及陪伴等方式,引發個案自我照顧動機,使勇敢面對疾病並努力學習自我照護能力,預防疾病再復發,藉由護理師功能的發揮協助個案走出陰霾。
