    Title: 冠狀動脈疾病患者血清中骨髓過氧化?及高敏感性C-反應蛋白的相關性研究

    Abstract: 冠狀動脈疾病(Coronary Artery Disease;CAD)是指因供給心臟血液和養分的冠狀動脈,產生了狹窄或阻塞,導致心肌損傷所造成的心臟疾病。根據過去的研究指出急性心肌梗塞(Acute Myocardial Infarction;AMI)發生的過程中,骨髓過氧化?(myeloperoxidase;MPO)扮演著非常重要的角色,當發炎反應發生時,白血球活化會將MPO釋放出來,而且MPO會透過自由基的產生損傷血管內壁而產生動脈粥樣硬化。由於動脈粥狀硬化是血管壁受損的慢性發炎疾病,所以(High sensitivity C-reactive protein;HS-CRP)在臨床上也常做為心血管疾病之組織損傷的篩檢工具。為了研究血清中MPO及HS-CRP的含量是否有助於AMI的評估與預防,以AMI病患為研究對象,分別在心導管手術前、術後24小時與術後7天這三個時間點收集病患血液檢體後,放入不含有抗凝劑的試管,分離並收集血清,再以MPO與HS-CRP的試劑套組進行檢測,得到血清中MPO與HS-CRP的數值。研究結果發現MPO及HS-CRP的濃度在心導管手術前分別為1539.97 (pmole/L)與0.59 (mg/dl),術後24小時為784.93 (pmole/L)與1.54 (mg/dl),而術後7天則為316.29 (pmole/L)與0.54 (mg/dl)。而心導管術後7天MPO的濃度比心導管手術前(P=0.021)及術後24小時(P<0.001)的濃度都低,具有統計上的差異;心導管術後7天後的HS-CRP濃度則比術後24小時的濃度高,亦具有統計學上的差異(P=0.021)。根據本次研究的結果,血清中MPO與HS-CRP的濃度與AMI之間具有相關性,由於AMI為嚴重的CAD臨床表現,因此可透過持續追蹤血清中MPO與HS-CRP的濃度作為CAD預後的評估與預防保健的指標。
    Coronary artery disease (CAD) is defined as blockages of the coronary arteries, which limits blood flow to certain areas of the heart muscle. This lack of blood flow can result in the muscle dying off as in myocardial infarction or heart attack. According to previous studies that myeloperoxidase (MPO) plays an important role in acute myocardial infarction (AMI) . MPO, a highly abundant leukocyte enzyme, is secreted by leukocytes during inflammation. MPO as a component of human vascular lesions, because of this heme protein can generate reactive species that damage lipids and proteins, myeloperoxidase may contribute to atherogenesis by catalyzing oxidative reactions in the vascular wall. High-sensitivity C-reactive protein (HS-CRP) measurements are useful in clinical practice for the diagnosis and treatment monitoring of many acute and chronic inflammatory conditions. HS-CRP is most often used to help predict a healthy person's risk of cardiovascular disease. To evaluate the role of plasma MPO and HS-CRP as predictive factors for the evaluation and prevention of AMI, we collected and studied clinical specimens from 17 AMI patients with Cardiac Catheterization. Plasma levels of MPO and concentrations of HS-CRP were determined by using a commercial kit. And examined at different time: before Cardiac Catheterization, post-24 hrs of Cardiac Catheterization and post-7 days of Cardiac Catheterization. Data showed that the average levels of MPO and HS-CRP from the AMI patients before Cardiac Catheterization is 1539.97 (pmole/L) and 0.59 (mg/dl); post-24 hrs of Cardiac Catheterization is 784.93 (pmole/L) and 1.54 (mg/dl); post-7 days of Cardiac Catheterization is 316.29 (pmole/L) and 0.54 (mg/dl). The average levels of MPO from the AMI patients post-7 days of Cardiac Catheterization were lower than before Cardiac Catheterization (P=0.021) and post-24 hrs of Cardiac Catheterization (P<0.001); and the concentrations of HS-CRP from the AMI patients before Cardiac Catheterization were lower than post-7 days of Cardiac Catheterization (P=0.027) and the concentrations of HS-CRP in post-7 days of Cardiac Catheterization were higher than post-24 hrs of Cardiac Catheterization (P=0.021). These findings support a potential role for MPO and HS-CRP as an inflammatory marker in CAD and demonstrate the prognostic value of plasma MPO and HS-CRP in the onset of AMI.
