    Title: 早產兒父母的不確定感與因應行為之探討
    A Study on the Uncertainty and Coping Strategies of Preterm Infants Parents

    Abstract: 本研究目的在探討早產兒父母的不確定感與因應行為,採相關性研究設計法之結構式問卷調查,民國94年12月1日至95年4月18日於中部某醫學中心,以方便取樣選擇130位早產兒父母為樣本,研究工具包括Mishel雙親不確定感認知量表及Jalowiec家屬因應策略量表。結果顯示:早產兒父親不確定感總平均分數82.74,因應行為總平均分數108.02;早產兒母親不確定感總平均分數83.26,因應行為總平均分數104.43,早產兒父母不確定感前五名依次為「我不清楚孩子將來會如何」、「我很難確定要用多久的時間,才能學會照顧孩子」、「我不清楚孩子還要承受多少苦」、「我不知道我的孩子那裡不舒服」、「孩子出院後,我不清楚應如何照顧他/她」,早產兒父母因應行為前五名依次為「我期望事情會好轉」、「我盡力尋找關於照顧孩子的資料」、「我告訴自己不用擔心,任何事情都會圓滿解決的」、「我嘗試更進一步了解所面臨的問題」、「我嘗試去維持目前所能掌握的情況」。男性早產兒、單胞胎、疾病嚴重度越高、出生週數越少、出生體重越低者其父母之不確定感較高,早產兒父母不確定感與採情感取向的因應行為呈正相關,折衷家庭較常使用情感取向的因應行為,早產兒父母不確定感與採問題取向的因應行為呈負相關,父親及教育程度越高者較常使用問題取向的因應行為。研究建議:護理人員需主動了解父母不確定感因素,協助父母獲得認知行為策略及相關訊息,以加強家庭和社會生活的適應,提昇護理服務品質。
    The purpose of this study was to explore the relationship between uncertainty and coping strategies of preterm infant parents. This was a cross-sectional design and purposive sampling was conducted. One thirty parents of preterm infants were recruited since December 1, 2005 to April 18, 2006 in a medical center in Taichung City. Structured questionnaires were utilized, such as:Mishel’s Parent Perception Uncertainty in illness (PPUS) and Jalowiec Coping Scale(JCS). The theoretical framework was designed based on uncertainty theory developed by Mishel. The PPUS mean score of the preterm infants’ fathers was 82.74 for total uncertainty and JCS mean score was 108.02 for total coping. The PPUS mean score of preterm infants’ mothers is 83.26 for total uncertainty and JCS mean score is 104.43 for total coping. The top five uncertainty items of preterm infants’ parents were: ‘It is not clear what is going to happen to my child.’ ‘It is difficult to determine how long it will be before I can care for my child by myself.’ ‘It is unclear how bad my child’s pain will be. ’‘I don’t know what is wrong with my child. ’‘It’s vague to me how will I manage the care of my child after he/she leaves the hospital.’‘ The top five coping behavior items of preterm infants’ parents were: ‘Hope that things will get better’ ‘Try to find out more about the situation so you can handle it better.’‘ Don’t worry about it, everything will probably work out fine. ’‘Try to find the purpose or meaning of the situation.’ ‘Try to maintain some control over the situation. ’ The preterm infants’ parents uncertainty were high score associated with mean of preterm infants and singleton birth and preterm infants of illness severity and lower gestational age and lower birth weight. Positive correlations were noted between the scores of uncertainty and emotional-focused strategies of preterm infants’ parents. Stem Family tended to emotional-focused strategies. Negative correlations were noted between the scores of uncertainty and problem-focused strategies of preterm infants’ parents. Father and high education tended to use problem- focused strategies. Pediatric nursing staffs can apply the findings to help preterm infants’ parents to use cognitive coping behaviors to handle uncertainty feelings and to enhance family members to adapt social life and improve quality of care.
