    Title: 青少女之生育結果及育兒狀況研究;Births to adolescent mothers : obstetric outcomes and child health and development

    Abstract: 根據既往多數研究指出,青少年母親及其構成的家庭之特性會引起不良生育結果及新生兒生長發育遲緩或照顧不週的問題。但這些不良結果為年齡單一因素造成,或是由多重因素影響仍有爭議。本研究屬個案對照研究,以台中市青少女母親之育兒為病例組,正常育齡母親之育兒為對照組,其主要目的在分析青少女及其構成的家庭對所生育嬰兒的影響,包括出生結果、生長情形、生活照顧及發育狀況。
    研究結果發現青少女懷孕生育的新生兒之低體重兒的比率較高,當進行新生兒出生體重有關之多變項迴歸分析時,在控制其他變項後發現母親懷孕時增加體重及懷孕時住院次數與新生兒是否為低體重兒呈顯著的相關。早產兒及先天性疾病的比率較高,但不具統計學上意義。而出生後生長之身高、體重、頭圍小於十個百分位之比率較高,生活的照顧包括重要疾病頻率、意外傷害頻率均較多,哺育母乳率較低,但也沒有到達統計上的意義。而發育之粗動作、細動作、語言、身體處理及社會性的延遲上沒有顯著差別。
    本研究發現青少女懷孕生育的結果及其的健康和發展與正常育齡者間有統計學上顯著的差別,但其長期預後值得進一步探討。

    Obstetric Outcomes and Child Health and Development
    Abstract
    An increased risk of adverse outcomes of teenage pregnancy as indicated by newborn consequence and developmental status has been well-known﹒The study investigate the status of adolescent pregnancy outcomes as well as the growth and development of children born to adolescent mothers as compared to those born to adult mothers﹒
    The sample is composed of adolescent mothers and mothers aged 20-29 years old and their firstborn children born in Taichung city in 1997﹒Three results were examined by a self-administered questionnaire to collect the information on birth outcomes and growth and development of children born to adolescent and adult mothers﹒There is no significant difference between children of teenage and adult mother in terms of the prevalence of prematurity and congenital diseases, but the children of teenage mother has higher prevalence of low birth weight than those of adult mothers﹒ Multiple logistic regression was used to analyze the adjusted risk factor for low birth weight﹒It’s found that weight gain and admission frequency during pregnancy in mothers were significant risk factors for low birth weight﹒There is no difference in the rate of failure to thrive including weight, height, and head circumference between two groups though a higher poor prognosis existed in children born to adolescent mothers﹒A long-term follow-up study for the adolescent mothers and their children was recommended﹒
