    Title: 照護一位接受腎臟移植患者之護理經驗

    Abstract: 本文描述一位腎移植術後患者的護理經驗,個案為慢性腎衰竭患者,經歷近十年的透析日子後,接受腎臟移植手術。筆者於95年1月18日至95年2月5日護理期間,利用Gordon 11項功能性健康型態評估個案,藉由觀察及訪談方式收集資料後發現個案有:焦慮、高危險性感染、睡眠型態紊亂及健康尋求行為等護理問題。筆者在護理過程中採傾聽、同感心及鼓勵予以心理支持,主動關懷身體舒適、協助維持生理功能及讓家人支持使其重獲信心,並給予腎移植術後之日常照護、飲食、服藥及併發症衛教等護理,而個案於醫療團隊支持下順利出院,並能獲得良好的居家照顧。希望藉此篇個案報告以提供護理人員照顧類似病患之參考,以提升護理品質。
    This report describes the experience of providing nursing care to a patient with end- stage kidney disease who had undergone regular dialysis for 10 years prior to receiving a kidney transplantation. Using the Gordon 11-item Functional Health Pattern Assessment, the author did a complete case assessment between Jan. 18, 2006 and Feb .05, 2006. Information was collected through a series of observations and interviews. During the nursing care period, the patient was found to suffer from anxiety, severe infection, sleep disturbance, and irritability. The primary nurse used active listening and empathy skills to support the patient, provided clinical care to maintain the patient’s physical function, provided information regarding post-operative care, and involved family members in the caring process. Post kidney transplantation education for the patient included information about daily self-care, medications, complications and preventative care. In addition, we helped the patient face the challenges of presented by the whole therapeutic course and to obtain adequate home care and good social adaptation. By sharing these experiences, we hope to improve the nursing experience and quality of care of patients receiving kidney transplantations.
