Cerebral sparganosis is an extremely rare intracranial parasitic infectious disease and its true incidence might be underestimated. A 64-year-old man presented a one-year history of episodic general tonic-clonic seizure and progressive left leg weakness. MRI of the brain confirmed the presence of a lesion of the right precentral gyrus. Subsequent stereotactic biopsy revealed plerocercoid larva or sparganum. We were able to treat this patient successfully using this stereotactic technique without exising the whole mass. So we suggested the key is to remove the parasite not excise the whole granuloma mass. This technique will be especially effective especially when the lesion is deep-seated or located in a sensitive cortical area.
